Dublin, OH – November 16, 2025 – A tragic accident on Hyland Croy Road resulted in the death of 17-year-old Claudia Phan and left four others injured on Sunday evening, according to the Ohio State Highway Patrol.
What Happened
The accident occurred around 7:24 p.m. near Johnston Drive when Phan, driving a 2016 Kia Forte northbound, crossed the centerline and collided with a southbound 2017 Ford Explorer. The impact caused both vehicles to veer off the road, with the SUV, driven by 37-year-old Jason Lyle from Lewis Center, coming to a stop on the right side of the road, along with Phan’s Kia.
Phan sustained life-threatening injuries and was tragically pronounced dead at the scene by the Union County Coroner’s Office. Lyle, who suffered serious injuries, was rushed to Riverside Methodist Hospital. His wife, 34-year-old Ashley Lyle, who was sitting in the front passenger seat, was also transported to the hospital but with less severe injuries. Two young passengers in the Ford Explorer were taken to Nationwide Children’s Hospital, where they were treated for minor injuries.
