LANCASTER COUNTY, NE — The Lancaster County Sheriff’s Office (LSO) has confirmed the identity of the victim in a tragic crash that occurred east of Waverly on Thursday, November 20, 2025. Fernando Ortiz, 86, from Waverly, tragically lost his life in the accident, which took place at the intersection of N. 162nd Street and Bluff Road.

    What Happened

At 9:14 a.m. on Thursday morning, emergency responders from Waverly Fire and Rescue were dispatched to the scene of a two-vehicle collision. According to the LSO, a dump truck belonging to the Lancaster County Engineering Department was traveling south when it collided with a red vehicle heading west. The impact caused the red vehicle to be forced through the intersection and into a nearby ditch.

Despite the prompt efforts of emergency medical personnel, Ortiz, who was a passenger in the red vehicle, was declared dead at the scene due to the severity of his injuries. His spouse, who was the driver of the red vehicle, sustained serious injuries and was transported to a nearby hospital, where she remains in stable condition.

    Who Was Fernando Ortiz?

Fernando Ortiz was well-known in his community as a kind and caring individual. At 86, he had lived a life filled with dedication to his loved ones and community. His sudden passing has left a void in the hearts of his family, friends, and neighbors. Ortiz’s wife, who was seriously injured in the crash, is deeply affected by the loss of her husband. The couple’s dog, who was also in the vehicle at the time of the accident, was taken to an animal clinic in Waverly for treatment.

    Investigation and Findings

Capt. John Vik of the LSO confirmed that the dump truck driver failed to stop at the intersection’s stop sign due to the foggy conditions present at the time. The truck had been spreading gravel on the road earlier in the day. The driver of the dump truck is cooperating fully with the ongoing investigation.

LSO, in collaboration with the Nebraska State Patrol, is continuing to investigate the circumstances of the crash. Authorities are looking into whether any citations will be issued in connection with the incident.

    Impact on the Community

The tragic accident marks the fourth traffic-related fatality in Lancaster County over the past two weeks, a sobering statistic that has left many in the community shaken. As the investigation continues, residents of Lancaster County are reflecting on the importance of road safety, particularly in adverse weather conditions, to prevent future accidents and fatalities.

    What Happens Next

The LSO and the Nebraska State Patrol will continue their investigation into the crash. Any updates or new developments in the case will be shared with the public as they become available.
